Where Hip Pain Really Comes From
A common misconception is that it always comes from the hip joint itself. In reality, many cases are influenced by what’s happening in the lower back, pelvis, muscles, or other surrounding structures.
That’s why simply resting or stretching the hip doesn’t always solve the problem.

Some of the most common contributors include:
- Tight or overworked muscles
- Restricted movement in the hip, pelvis, or lower back
- Poor movement patterns and posture
- Wear and tear within the joint
- Repetitive strain or overuse
- Compensation caused by another painful or restricted area
At Optimal Health Chiropractic, we look at how your hip, pelvis, and lower back are working together—not just where you feel pain—to identify what is contributing to your symptoms.
Types Of Hip Injuries & Conditions We Treat
Hip Bursitis – Inflammation of the fluid-filled sacs that cushion the outside of the hip. It commonly causes tenderness and pain while walking, climbing stairs, or lying on the affected side.
Hip Flexor Strains – Tight, overworked, or injured muscles at the front of the hip can cause pain when lifting your knee, climbing stairs, getting out of a chair, or extending your leg behind you.
Labral Irritation – Irritation or damage involving the cartilage around the hip joint may cause groin pain, clicking, catching, stiffness, or a feeling of instability.
Hip Arthritis – Wear and tear within the joint can lead to pain, stiffness, and reduced range of motion, especially after sitting or first getting up in the morning.
Muscle And Tendon Injuries – Strains, tendinitis, and repetitive-use injuries can cause soreness, weakness, and pain when walking, exercising, or moving the leg.
Restricted Hip Or Pelvic Movement – When the joints of the hip or pelvis are not moving properly, surrounding muscles may compensate and place additional stress on the painful area.
Referred Pain From The Lower Back – Problems within the lower spine can create pain that is felt in the buttock or hip even when the hip joint itself is not injured.
Effective Chiropractic Treatments for Hip Pain
The discomfort does not always come from one isolated structure. Restricted movement in the lower back or pelvis can change how you walk and place additional stress on the hip. An injured tendon or muscle can also cause you to compensate, placing more strain on nearby joints.
During your evaluation, we’ll examine your range of motion, identify which movements trigger pain, and determine whether the problem begins in the hip, pelvis, lower back, or surrounding soft tissues.

Your treatment plan may include:
Manual Adjustments – Restricted movement in the lower back, pelvis, or hip can interfere with the way weight is distributed when you stand and walk. Carefully applied adjustments help restore joint motion, improve overall function, and reduce mechanical stress around the painful area. Treatment will be adapted to your condition and comfort level.
MLS Laser Therapy – Inflamed muscles, tendons, and bursae can cause pain when walking, climbing stairs, or lying on your side. MLS Laser Therapy uses targeted light energy to help reduce inflammation, improve circulation, and support soft-tissue healing. It may be beneficial for hip strains, tendinitis, bursitis, and other soft-tissue injuries.
SoftWave Therapy – Chronic hip pain may involve tendons and other tissues that have not fully healed. SoftWave Therapy increases blood flow and stimulates the body’s natural repair response within the targeted area. It may help reduce persistent pain, improve tissue healing, and make movement more comfortable.
Is Your Pain Actually Sciatica?
Pain felt in the hip or buttock does not always come from the hip joint. Sciatica occurs when a nerve root in the lower back becomes compressed or irritated, causing symptoms along the path of the sciatic nerve.
Sciatic nerve pain may begin in the lower back or buttock and travel into the thigh, calf, or foot. This differs from many hip conditions, which usually cause pain around the groin, outside the hip, or directly over the joint.
Your hip pain may be related to sciatica if you experience:
- Burning or electrical pain in the buttock
- Pain that travels down the back of one leg
- Numbness or tingling in the leg or foot
- Weakness in the affected leg
- Pain that becomes worse while sitting
- Lower back pain along with hip or leg pain
